LIABILITY RELEASE FORM

Eddie Sedlock (hereinafter referred to as “Coach”) and Identity Exchange LLC (hereinafter referred to as “the Company”) is not an employee, agent, lawyer, doctor, therapist, registered dietician, or psychotherapist of the Client. Client understands that this Coach has not promised, and shall not be obligated to act as a therapist providing psychoanalysis, psychological counseling or behavior therapy to the Client. Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach. The Coach and Company makes no guarantees or warranties, express or implied. In no event will the Coach or Company be liable to the Client for consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s and Company’s entire liability, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the Company for all services rendered up until the agreed ending date.

Permission to Record and Use for Training Purposes

The Client hereby grants permission to the Coach and the Company to record coaching sessions. These recordings may be used solely for the purpose of training, supervision, and quality improvement within the Company. The recordings and Client’s identity will be kept confidential and will not be shared with any third parties without the Client's explicit written consent.

Coaching with Eddie Sedlock

God remolded Eddie's heart from one wounded and hardened by the world into a passionate servant, dedicated to impacting the nations within the workplace.

Eddie discovered Identity Exchange through the transformative book Living Fearless by Jamie Winship. Engaging in coursework and building relationships within the Identity Exchange community, he evolved into a compassionate co-laborer, fully embracing his identity as a "Compassionate Foot Soldier and Torchbearer for Christ, trusted in the sacred heart space." Eddie is committed to walking alongside individuals on their personal paths of discovery and helping them know their true identity through the intimacy and creativity of God.

Now residing in Granville, Ohio, Eddie enjoys life with his wife, Emily, his smarter and better half of 25 years. Together, they are proud parents of three children.
