LIABILITY RELEASE FORM

Doug Bercume (hereinafter referred to as “Coach”) and Identity Exchange LLC (hereinafter referred to as “the Company”) is not an employee, agent, lawyer, doctor, therapist, registered dietician, or psychotherapist of the Client. Client understands that this Coach has not promised, and shall not be obligated to act as a therapist providing psychoanalysis, psychological counseling or behavior therapy to the Client.

Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach. 

The Coach and Company makes no guarantees or warranties, express or implied. In no event will the Coach or Company be liable to the Client for consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s and Company’s entire liability, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the

Company for all services rendered up until the agreed ending date.

Coaching with Doug Bercume

 

Doug Bercume has over 25 years of combined experience working in a variety of sectors.  His experience includes Strength and Conditioning Coach, Professor of Kinesiology, medical device sales management, Special Agent, and consulting through various government entities in the criminal justice and military communities. From violent criminals to government officials, college students to surgeons, CEOs to professional athletes, Doug has learned that we are all on a journey of wholeness.

In the early years of his career, Doug discovered that he had a unique insight that helped people identify the obstacles that blocked their performance.  As clients sought help for their physical injuries and aid in their human performance, what they were truly seeking was always rooted in something deeper. During his employment as a Special Agent, Doug met Jamie & Donna Winship. As he learned more about living in a true identity, he realized that this was the key to health and wellness.

Doug grew up working various construction jobs in the mountain of Colorado.  His time wandering through the Rockies helped shape his love of the outdoors and adventure. Doug utilizes his background to coach youth sports. He has three children and lives in the Midwest.
