LIABILITY RELEASE FORM

Catherine Johnson (hereinafter referred to as “Coach”) and Identity Exchange LLC (hereinafter referred to as “the Company”) is not an employee, agent, lawyer, doctor, therapist, registered dietician, or psychotherapist of the Client. Client understands that this Coach has not promised, and shall not be obligated to act as a therapist providing psychoanalysis, psychological counseling or behavior therapy to the Client.

Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach.

The Coach and Company makes no guarantees or warranties, express or implied. In no event will the Coach or Company be liable to the Client for consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s and Company’s entire liability, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the Company for all services rendered up until the agreed ending date.

Coaching with Catherine Johnson

Catherine Johnson has been with Identity Exchange since its inception. Her core identity is a Trail-Blazing, Atmosphere Changing, Trusted Child of God.

Catherine is a Spiritual Director with a private practice, Never Alone Spiritual Direction LLC. She earned her MA in Spiritual Formation at Portland Seminary and her BA at Stanford University in International Relations.

Catherine resides in the Portland, OR area and enjoys laughing, reading, running, gardening, being with her family and dear friends, and most of all experiencing the love of the Trinity.
